A Comparative Study of the Effectiveness of Mindfulness-Based Stress Reduction and Compassion-Focused Therapy on Psychosomatic Symptoms and Physical Health in Patients with Hypertension

Objective: Hypertension is one of the major risk factors for cardiovascular disease, and its management requires the modification of psychological factors and health-related behaviors. The present study aimed to compare the effectiveness of Mindfulness-Based Stress Reduction (MBSR) and Compassion-Focused Therapy (CFT) on psychosomatic symptoms and physical health in patients with hypertension.
Methods: This quasi-experimental study employed a pretest–posttest and follow-up design with a control group. The study population consisted of all patients with hypertension in Kashan in 2025. A total of 45 patients were selected through convenience sampling and randomly assigned to three equal groups: MBSR (n = 15), CFT (n = 15), and a control group (n = 15). Participants in the intervention groups received eight weekly sessions based on the respective therapeutic protocols, whereas the control group received routine medical care only. The research instruments included the Lacourt Psychosomatic Symptoms Questionnaire, the Physical Health Questionnaire, and the Life Stress Questionnaire. Data were analyzed using multivariate analysis of covariance (MANCOVA) and mixed-design analysis of variance with repeated measures in SPSS.
Results: The results of the analysis of covariance indicated that both MBSR and CFT had significant effects on reducing psychosomatic symptoms and improving physical health compared with the control group. The group effect sizes for psychosomatic symptoms (0.67) and physical health (0.65) indicated a large practical effect of the interventions. Furthermore, the results of the repeated-measures analysis and pairwise comparisons at posttest and follow-up indicated the relative stability of the therapeutic gains associated with both interventions at follow-up. The pattern of the proposed scores suggested more favorable mean scores for the mindfulness group on some indicators; however, a direct pairwise comparison to establish a definitive superiority requires analysis of the final empirical data.
Conclusion: The findings suggest that both MBSR and CFT are effective approaches for reducing psychosomatic complaints and improving physical health in patients with hypertension, potentially through modulating physiological responses to stress, reducing self-criticism, and promoting self-care behaviors. These psychological interventions are recommended as complementary approaches alongside routine medical care in healthcare settings.
